What can I expect from Your Advance Directive:
A live, virtual visit with a Licensed Clinical Social Worker (LCSW) who will walk you through the completion of your health care proxy form and Advanced Directive (also known as a living will or healthcare directive). Rest easy that your preferences for healthcare are accurately captured in your Advance Directive and that it is legally compliant in your State of residence.
Who Your Advance Directive is not for:
If you prefer to go at this on your own, this service is not for you. Though time-consuming and potentially stressful, it is possible to complete your own Advance Directive for free with online resources. While an Advance Directive can also be completed during a hospitalization or during a primary care visit, many physicians cannot dedicate the necessary amount of time to have these important conversations. Social workers are truly the experts in facilitating these conversations and helping you complete your Advance Directive.
How much will Your Advance Directive cost:
Create up to two Advance Directives for $250. This includes a 1.5-hour virtual visit with a Social Worker. Great value for a couple or an adult child helping their parent complete their advance directive and health care proxy (medical power of attorney). We do not accept insurance.
